Jon-Erik Hexum was a fast upcoming Hollywood actor and a model. He died at the age of 36.
On set on October 12, 1984, one of his scenes required him to stack a 44 Magnum handgun with blank rounds.
There was a postponement in recording, and Hexum became impatient. He retorted to playing with the short gun by removing the blank rounds he had early loaded. He spurned the weapon around, turned it to his head and fired. Unknown to him, the blank rounds contain paper waddling which discharged when fired and this can be very dangerous when fired at a close range.
So, the force from the weapon was sufficient enough to break a little piece of his skull, and push it into his brain that caused a serious hemmorrhaging.
He was quickly sent to the hospital. He went through five hours of brain medical procedure that yielded no result. He was declared “brain dead” and after six days, he was declared dead after being removed from life support.
The main positive outcome of his death was that his organs were donated to at least three people.
